The 2016 Conflict of Interest (COI) recertification process will begin shortly. Every faculty member, postdoc, staff member, and other applicable covered person (as defined in the COI policy) is required to submit an annual disclosure form reflecting all actual, potential, or perceived conflicts of interest and/or commitment. This recertification process is done electronically.
On Wednesday, October 19, you will receive an email notification containing a link to the COI web page to complete the annual recertification process. Prior to completing the disclosure, you are encouraged to read the Policy, Charter, and Administrative Procedures as your electronic signature at the end of the recertification process confirms that you have read and understand these documents.
The automated recertification process includes the ability to save information already completed for future review, revision, or completion prior to your electronic signature and final submission. If you save information in the disclosure form without submitting it, the next time you log into the site just click on ‘View My History’ to access the form.
The due date for submission is no later than Wednesday, November 2. If you fail to submit the disclosure by the deadline, reminder emails will be sent to your respective manager and/or vice president.
